Position Overview:

Become a vital member of Unity Care NW, a distinguished healthcare provider dedicated to serving the community. As a Primary Care Provider, you will deliver comprehensive medical services to a varied patient demographic within a supportive and collaborative atmosphere. Our facilities provide a range of services including primary medical care, behavioral health support, and dental services, all centered around the needs of our patients.

Key Responsibilities:

  • Engage in 36 hours of patient interaction weekly, delivering primary care.
  • Work closely with patients and their families to ensure effective care.
  • Incorporate behavioral health services for a comprehensive treatment approach.
  • Facilitate case management to link patients with essential community resources.
  • Opportunity to participate in the Communicable Disease Program.

Qualifications:

  • Completion of an accredited Nurse Practitioner or Physician Assistant program.

  • Experience working with diverse patient groups and complex health issues.
  • Familiarity with Electronic Medical Records systems.
