Become a vital member of Unity Care NW, a distinguished healthcare provider dedicated to serving the Whatcom County community. As a Primary Care Provider, you will deliver comprehensive medical services to a varied patient demographic within a supportive and collaborative setting. Our facilities provide an array of services including primary medical care, behavioral health support, and dental services, all centered around the needs of our patients.
Key Responsibilities:
- Engage in 36 hours of patient interaction weekly, delivering primary care services.
- Work closely with patients and their families to ensure effective care management.
- Incorporate behavioral health services to promote a holistic healthcare experience.
- Facilitate case management to link patients with essential community resources.
- Opportunity to participate in the Communicable Disease Program.
