Job Description

Job Summary:

CorsoCare is seeking a skilled Healthcare Navigator to join our team. As a Healthcare Navigator, you will be responsible for providing patient support, coaching, and ensuring services are in place for a successful transition home.

Key Responsibilities:

  • Patient Support: Provide patient support, coaching, and ensure services are in place for a successful transition home.
  • Relationship Building: Build and expand relationships with physicians, hospitals, skilled nursing facilities, senior agencies, and other senior communities.
  • Education and Support: Provide educational presentations and materials, support groups, needs assessments, advanced care planning, and other support such as patient calls and supporting the health care help line.
  • Communication: Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Problem-Solving: Identify and address patient needs, and develop solutions to ensure successful care transitions.

Requirements:

  • Experience: 3-5 years of healthcare experience preferred, but not required.
  • Education: Social work or clinical liaison background preferred but not required.
  • Skills: Strong technical skills in Microsoft Office Suite (Word, Excel, PowerPoint, TEAMS, Outlook), strong presentation and public speaking skills, good communicator, able to articulate product lines and services, sales skills, solutions orientated, thrives in a fast-paced environment, proactive, organized, and energetic.
