About the Role:

We are seeking a skilled Healthcare Navigator to join our team at Alternate Solutions Health Network. As a key member of our team, you will play a vital role in ensuring seamless transitions for our patients from hospital to home.

Key Responsibilities:

  • Identify potential patients and assess their eligibility for home care services.
  • Collaborate with hospital and facility personnel to facilitate discharge planning and coordinate post-acute care services.
  • Coordinate healthcare services as ordered by attending physicians and ensure timely delivery of ancillary services.
  • Serve as a resource for patients and their families, providing education and support throughout the care transition process.
  • Maintain accurate and comprehensive clinical documentation in accordance with agency protocols and regulatory requirements.
  • Participate in care integration meetings and build strong relationships with patients, families, healthcare providers, and other stakeholders.

Requirements:

  • Associate's degree or equivalent experience required; Bachelor's degree preferred.
  • Minimum of two years' experience in healthcare or a related field; experience in care coordination or case management a plus.
  • Valid driver's license and reliable transportation required.
  • Ability to lift up to 25 pounds and perform other physical demands of the job.
