Job Summary

We are seeking a highly skilled and compassionate Patient Care Navigator to join our team at Alternate Solutions Health Network. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions for our patients from hospital to home care settings.

Key Responsibilities
  • Identify and Assess Patient Needs: Conduct thorough assessments of patients' medical and social needs to determine eligibility for home care services.
  • Coordinate Care Transitions: Collaborate with hospital and facility personnel to ensure smooth transitions of care, including attending discharge rounds and coordinating with healthcare providers.
  • Develop and Implement Care Plans: Work with patients, families, and healthcare providers to develop and implement personalized care plans that meet their unique needs and goals.
  • Provide Education and Support: Educate patients and families on home care services, provide emotional support, and address any concerns or questions they may have.
  • Communicate Effectively: Maintain open and effective communication with patients, families, healthcare providers, and other stakeholders to ensure seamless care coordination.
Requirements
  • Education and Experience: Associate's degree in a healthcare-related field and a minimum of two years of experience in healthcare or a related field.
  • Licensure and Certification: Preferred licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Physical Therapist, Physical Therapist Assistant, or Licensed Social Worker (LSW).
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong problem-solving and critical thinking skills.
