Position Summary:
The Registered Nurse (RN) Case Manager provides essential palliative care to patients facing terminal illnesses and their families, adhering to a medically sanctioned care plan. This role requires adeptness in triaging emergencies, both in-person and via telephone, while effectively conveying patient needs to the appropriate hospice team members.

Key Responsibilities:
- Establishes and nurtures productive relationships with colleagues, patients, and other healthcare professionals.
- Implements a personalized Plan of Care developed by the interdisciplinary team for each patient.
- Coordinates comprehensive care for assigned patients, integrating all hospice disciplines.
- Collaborates with the hospice interdisciplinary team to create and uphold individualized care plans and treatment objectives.
- Collects, reports, and evaluates patient care data, adjusting care as necessary.
- Provides detailed updates on patient conditions to the Hospice Medical Director and interdisciplinary team members.
- Immediately notifies the Hospice Medical Director and interdisciplinary team of any changes in patient status.
- Reports significant observations and relevant information to the physician or Director of Nursing.
- Understands and applies fundamental principles of palliative care and pain management within the professional scope.
- Adheres to all documentation policies and procedures, ensuring timely and legible records.
- Utilizes the appropriate chain of command for reporting and decision-making.
- Supervises and assigns tasks to Certified Nurse Aides and Licensed Practical Nurses.
- Participates in Interdisciplinary Team Meetings and contributes to Performance Improvement Activities.
- May pronounce death in accordance with established physician orders.
- Coordinates with the coroner and/or the chosen mortuary for the removal of the deceased.
- Offers emotional and spiritual support, making referrals to other team members as needed.
- Addresses and assesses emotional distress related to terminal illness.
- Facilitates communication among patients, families, and the hospice team.
- Identifies caregiver stress and provides necessary respite care or support.
- Educates patients and families on completing Advance Directives upon request.
- Delivers personal care as directed by the Interdisciplinary Team, Hospice Physician, and Director of Clinical Operations, which may include:
- Nursing assessments and reassessments
- Medication management and administration
- Providing comfort measures
- Insertion and maintenance of foley catheters
- Insertion and management of IV catheters for pain control
- Application and adjustment of oxygen and other invasive lines as ordered
- Monitoring vital signs
- Assisting with patient mobility and transfers
- Complying with the State of Nevada Nurse Practice Act
- Following established clinical care policies and guidelines

- Must possess the physical strength and endurance for standing, walking, lifting, bending, and pushing consistently.
- Utilizes durable medical equipment safely as needed.
- Applies appropriate body mechanics for personal and patient safety.
- Engages in tasks with potential exposure to blood and body fluids, adhering to safety protocols.
- Employs personal protective equipment (PPE) as required.
- Follows emergency action plans in case of fire or disaster.
- May require travel to client meetings or patient homes.
- Typically works in a quiet environment.

Qualifications and Experience:
- Must hold a current Registered Nurse license from the State Board of Nursing in Nevada.
- Preferred: 1 year of experience in hospice, home health, or geriatric nursing.
- Successful completion of background checks and verification of previous employment.
- Proficient in reading, writing, and understanding English.
- CPR and First Aid certification required.
- Ability to manage frequent interruptions and cope with emotionally charged situations.
- Availability for on-call hours, weekends, and holidays.
- Must have reliable transportation and current auto insurance.
