Position Summary:

The Palliative Care Nurse Practitioner is responsible for delivering compassionate care to patients and their families, adhering to a medically sanctioned care plan. This role operates under the guidance of the interdisciplinary team and the Chief Medical Officer or Hospice Physician.

Responsibilities:

The Nurse Practitioner will:

  1. Develop and maintain positive relationships with colleagues, patients, and community professionals.
  2. Implement individualized care plans as determined by the Interdisciplinary Team for each patient.
  3. Support the Chief Medical Officer and/or Hospice Physician in managing patient care by providing direction for medical orders and interventions aimed at enhancing patient outcomes.
  4. Collaborate with RN Case Managers to establish treatment objectives and interventions for patients.
  5. Collect, report, and analyze patient care data to inform care decisions.
  6. Conduct in-person visits for patients within designated timeframes.
  7. Assist the clinical team in family support by offering advanced medical insights regarding patient conditions and treatment alternatives.
  8. Provide comprehensive updates on patient status to the Chief Medical Officer, Hospice Medical Director, RN Case Managers, and other interdisciplinary team members as necessary.
  9. Report any unusual findings or significant observations to the appropriate team members.
  10. Understand and apply fundamental principles of palliative care and pain management within the scope of practice.
  11. Adhere to all documentation protocols and ensure timely and legible record-keeping.
  12. Execute physician and medication orders promptly, within the scope of practice.
  13. Utilize the appropriate chain of command for all communications.
  14. Oversee and delegate tasks to nursing team members as required.
  15. Engage in Interdisciplinary Team Meetings and other Performance Improvement Activities as needed.
  16. Complete death certificates in accordance with NRS guidelines.
  17. Provide necessary interventions and support related to terminal illness and bereavement.
  18. Offer emotional support and facilitate referrals to other team members as appropriate.
  19. Assess and address feelings of depression and anxiety associated with terminal illness for both patients and their families.
  20. Encourage communication among patients, families, and the hospice team.
  21. Identify caregiver stress that may require respite care or additional support.
  22. Act as a liaison between RN Case Managers, Admissions RN, and the Chief Medical Officer or Hospice Physician.
  23. Participate in evaluating the competencies of nursing staff and other nurse practitioners.
  24. Contribute to community education initiatives as requested by leadership.
  25. Maintain an up-to-date knowledge of pharmaceuticals used for symptom management, including pain, dyspnea, nausea, constipation, and agitation.
  26. Comply with Nevada Rule AB474.
  27. Ensure patients meet hospice eligibility criteria and recommend re-certification or discontinuation of services to the Chief Medical Officer or Hospice Physician as necessary.
  28. Consult with the Chief Medical Officer or Hospice Physician when uncertain about disease management, treatment options, or required interventions beyond the nurse practitioner's expertise.
  29. Perform other duties as assigned.

Physical and Safety Requirements:

  1. Must possess the strength and endurance for prolonged standing, walking, and sitting.
  2. Must be able to lift up to 25 lbs. occasionally.
  3. Utilize appropriate durable medical equipment safely as needed.

  5. Category 1 exposure risk to blood/body fluids; protective measures must be used during tasks with exposure potential.
  6. Utilize appropriate personal protective equipment as required.
  7. Follow the designated emergency action plan in case of fire or other disasters.
  8. Regularly exposed to fumes, airborne particles, work-related stresses, and driving hazards.
  9. May require travel to meet with clients, healthcare professionals, or visit patients' homes.
  10. Noise levels in the work environment are typically minimal.

Qualifications and Experience:

  1. Must hold a current license as a Family Nurse Practitioner or Pediatric Nurse Practitioner from the State Board of Nursing in Nevada.
  2. Preferred: 2 years of experience as a Nurse Practitioner.
  3. Preferred: Previous experience in hospice or palliative care settings.
  4. Must successfully complete a background check in accordance with NRS and NRS 179A.100, a pre-employment physical and drug screening, and verification of references and previous employment.
  5. Must be proficient in reading, speaking, writing, and understanding the English language.
  6. Maintain BLS Certification.
  7. Hold active APRN licensure with the Nevada State Board of Nursing.
  8. Maintain active NPI and DEA registration and provide this information to the employer.
  9. Maintain proper licensure and active status to prescribe both non-controlled and controlled substances through the Nevada State Board of Pharmacy.
  10. Maintain active enrollment in the Nevada State Prescription Monitoring Program.
  11. Must not be exempt from working with Medicare patients.
  12. Ability to manage frequent interruptions effectively.
  13. Must be able to cope with emotionally charged situations involving patients, families, and caregivers.
  14. Availability for prescheduled on-call hours, weekends, and holidays is required.
  15. Must possess reliable transportation, maintain current auto insurance, and hold a valid driver's license.
