Job Overview

Position Summary:

The Registered Nurse (RN) Case Manager is responsible for delivering comprehensive palliative care to patients facing terminal illnesses and their families. This role operates within a medically sanctioned care plan, ensuring effective communication and coordination among the hospice team to address patient emergencies both in-person and via telephone.

Key Responsibilities:

  1. Establishes and nurtures collaborative relationships with colleagues, patients, and community professionals.
  2. Implements individualized care plans developed by the interdisciplinary team for each patient.
  3. Oversees the overall care of assigned patients, coordinating various hospice services.
  4. Assists the interdisciplinary team in formulating and upholding personalized care plans and treatment objectives.
  5. Collects, reports, and evaluates patient care data to adjust care as necessary.
  6. Provides detailed updates on patient conditions to the Hospice Medical Director and team members as required.
  7. Immediately notifies the Hospice Medical Director and interdisciplinary team of any significant changes in patient status.
  8. Communicates unusual findings or observations to the physician or Director of Nursing.
  9. Demonstrates knowledge of palliative care principles and pain management within the defined scope of practice.
  10. Adheres to all documentation standards and completes records accurately and promptly.
  11. Utilizes the appropriate chain of command for reporting and decision-making.
  12. Supervises and assigns tasks to Certified Nurse Aides and Licensed Practical Nurses.
  13. Participates in interdisciplinary team meetings and quality improvement initiatives.
  14. Conducts pronouncement of death in accordance with established physician directives.
  15. Coordinates with the coroner and/or chosen mortuary for the removal of the deceased.
  16. Offers emotional and spiritual support to patients and families during their time of loss.
  17. Addresses and assesses emotional challenges related to terminal illness.
  18. Facilitates communication between patients, families, and the hospice team.
  19. Recognizes caregiver stress and arranges for respite care when necessary.
  20. Educates patients and families on completing Advance Directives upon request.
  21. Provides personal care as directed by the interdisciplinary team, which may include:
  • Nursing assessments and reassessments
  • Medication management and education
  • Comfort care interventions
  • Management of catheters and IV lines
  • Monitoring vital signs
  • Assisting with mobility and transfers
  • Adhering to state nursing regulations during all patient care activities

Physical and Safety Requirements:

  • Must possess the physical stamina for standing, walking, lifting, and bending regularly.
  • Utilizes appropriate medical equipment safely.
  • Employs proper body mechanics for personal and patient safety.
  • May encounter exposure to blood and body fluids; follows safety protocols.
  • Utilizes personal protective equipment as necessary.
  • Adheres to emergency action plans during crises.
  • May require travel to client or patient locations.
  • Typically works in a low-noise environment.

Qualifications and Experience:

  1. Must hold a current Registered Nurse license in the State of Nevada.
  2. Preferred experience of at least one year in hospice, home health, or geriatric nursing.
  3. Successful completion of background checks and verification of employment history.
  4. Proficient in reading, writing, and understanding English.
  5. CPR and First Aid certification required.
  6. Ability to manage frequent interruptions and emotionally charged situations.
  7. Availability for scheduled on-call hours, weekends, and holidays.
  8. Must have reliable transportation and valid auto insurance.
