The Case Manager RN is responsible for ensuring high-quality, patient-centered care by managing rehabilitation care to achieve optimal outcomes. This role involves coordinating efforts within the rehabilitation team and collaborating with the rehabilitation program director and facility case management director.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including an explanation of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the discharge planning evaluation and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Associate Degree in Nursing or Nursing Diploma required; Bachelor's Degree in Nursing preferred.
- 2+ years of experience in case management or 3+ years of experience in clinical nursing required.
- Current license as a Registered Nurse in the state(s) of practice according to law and regulation.
- InterQual experience and Certification in Case Management preferred.
- Basic Cardiac Life Support (BCLS) required.
