Position Overview

Role Summary:

The RN Clinical Coordinator is responsible for overseeing and delivering direct patient care within the Adult Hospice Program. This role includes supervising the Interdisciplinary Group members, ensuring effective communication of the hospice care philosophy to patients, families, healthcare providers, and community stakeholders.

Key Responsibilities:

  1. Implement and follow a personalized Plan of Care developed by the RN for each patient.
  2. Ensure adherence to patient rights, confidentiality, and abuse reporting protocols.
  3. Promote and maintain collaborative relationships among peers, professional staff, and other personnel.
  4. Provide foundational knowledge of palliative care and pain management within the Scope of Practice.
  5. Participate in scheduled staff meetings and training sessions.
  6. Comply with departmental dress code and personal grooming standards.
  7. Engage in Performance Improvement initiatives as required.
  8. Demonstrate reliability by maintaining punctuality and notifying of absences or delays.
  9. Exhibit problem-solving skills by assessing situations, considering alternatives, and seeking guidance when necessary.
  10. Manage time effectively to maximize productivity and efficiency, adjusting work schedules as needed.
  11. Commit to ongoing personal development and continuing education.
  12. Stay updated with all mandatory agency and regulatory requirements.
  13. Oversee budget management, staff hiring, training, and the development of policies and protocols.
  14. Complete assigned projects within designated timelines.
  15. Perform additional duties as assigned.

Additional Responsibilities:

  1. Conduct hiring, counseling, and disciplinary actions for staff as necessary.
  2. Assist in the creation and implementation of policies and procedures.
  3. Engage in complaint resolution processes as directed.
  4. Ensure compliance with all regulatory standards set by State, Federal, CHAPS, and other governing bodies.
  5. Contribute to the development of effective corrective action plans in response to surveys.
  6. Evaluate and conduct performance assessments of staff members.
  7. Oversee the availability of clinical services on a 24-hour basis.
  8. Manage clinical aspects of patient care effectively.
  9. Participate in the budgeting process as requested by the Director of Nursing.
  10. Ensure proper admission requirements for hospice services are met.
  11. Supervise staffing and scheduling for patient care.
  12. Delegate duties to multidisciplinary clinical personnel appropriately.
  13. Facilitate coordination of Interdisciplinary Team Meetings.
  14. Organize staff meetings and training sessions as necessary.
  15. Handle confidential information with care.
  16. Maintain consistent attendance and punctuality.

Physical and Safety Requirements:

  1. Must possess the physical strength and endurance for standing, walking, lifting, bending, and pushing regularly, handling weights between 10 and 50 lbs.
  2. Utilize appropriate durable medical equipment and personal protective equipment safely.
  3. Employ proper body mechanics for personal safety and patient care.
  4. Be aware of potential exposure to blood/body fluids and follow safety protocols.
  5. Adhere to emergency action plans in case of fire or other disasters.
  6. Regularly exposed to work-related stresses and driving hazards.
  7. May require travel to client meetings, patient homes, or healthcare professionals.
  8. Noise levels in the work environment are typically minimal.

Qualifications and Experience:

  1. Must hold a valid RN license with the State of Nevada Board of Nursing.
  2. A minimum of one year of experience as an RN in a hospice or home health setting is required.
  3. CPR/First Aid certification is mandatory.
  4. At least one year of relevant hospice experience is necessary.
  5. Must successfully complete a background check and verification of previous employment and references.
  6. Proficiency in reading, speaking, writing, and understanding the English language is essential.
  7. Excellent verbal and written communication skills are required.
  8. Ability to manage frequent interruptions effectively.
  9. Capable of handling emotionally charged situations involving patients, families, and caregivers.
  10. Availability for on-call hours, weekends, and holidays is required.
  11. Must be able to prioritize and manage medications, durable medical equipment, and supplies using the hospice-approved formulary.
  12. Reliable transportation, a valid driver's license, and current auto insurance are necessary.
