Position Summary

As a Referral Navigator, you will oversee the management of incoming referrals, ensuring timely and effective connections between clients and the appropriate community partners. This role requires proficiency in utilizing various technology platforms, including the Healthy Alliance Solution Suite.

Key Responsibilities

  • Oversee incoming referrals at the Referral Coordination Center, ensuring efficient connections between clients and community partners.
  • Maintain open communication with clients and referring organizations to facilitate referral acceptance and resolution.
  • Document all referral progress and actions taken in accordance with Network Standards.
  • Provide feedback on partner performance and escalate unresolved issues promptly.
  • Manage content and processes to enhance partner communication and marketing materials.
  • Engage in continuous learning to support new product rollouts within the Healthy Alliance Solution Suite.
  • Occasional local travel to partner sites and community events may be required.

Qualifications

Education

Associate degree in health, social services, or a related field preferred. Equivalent work experience may be considered.

Experience

  • 5+ years of relevant experience in a clinical, non-profit, or managed care setting preferred.
  • Strong understanding of health equity and social determinants of health.
  • Experience with call centers or working with diverse populations is advantageous.
  • Bilingual Spanish skills are preferred.
