The Clinic Nurse will be responsible for planning, coordinating, and managing daily nursing operations. This role involves supporting primary care providers to ensure patient adherence to treatment objectives through effective care coordination. In addition to clinical responsibilities, the RN will contribute to achieving administrative targets. Opportunities for professional advancement will be available through participation in educational initiatives, literature reviews, in-service training, and workshops.

  • Conducts timely patient telephone consultations and triage assessments with clinical expertise for urgent or drop-in care;
  • Accurately documents in the electronic medical record;
  • Utilizes various EMR systems to compile patient information;
  • Responds promptly to emergencies, delegating tasks clearly to clinic personnel;
  • Performs nursing assessments, implements care plans, and organizes follow-up care for primary care patients;
  • Educates patients on diverse topics such as HIV, diabetes, hypertension, asthma, nutrition, and specific diagnostic procedures;
  • Implements team-based medical management according to established protocols for patients with chronic conditions;
  • Provides wound care services under provider supervision and according to clinical guidelines on-site and as part of outreach efforts.
  • Engages in daily Quality Improvement initiatives;
  • Conducts initial and ongoing assessments of client health status, immunizations, and TB screenings based on established protocols;
  • Administers adult immunizations, medications, and performs phlebotomy in line with current guidelines;
  • Delivers culturally competent and equitable client care;
  • Communicates effectively with clients, clinic staff, and the broader community in a respectful manner;
  • Collaborates with healthcare team members to provide comprehensive primary care services;
  • Participates in daily pre-clinic meetings, identifying patients with high needs for the clinic session;
  • Assumes clinical responsibilities of non-licensed staff as needed;
  • Conducts intake assessments for new patients, efficiently triaging based on urgency and complexity to the appropriate Primary Care Provider;
  • Links patients to relevant in-house and external medical, behavioral, and social services;
  • Oversees the vaccine management program in collaboration with public health authorities;
  • Maintains precise records of vaccines and therapeutic injection inventory, monitoring expiration dates and ordering supplies as necessary;
  • Manages administrative aspects of care coordination, including prior authorizations and specialty referrals;
  • Supervises clinic medication inventory and emergency supplies;
  • Processes routine medication refills promptly and prepares prior authorizations as per established workflows;
  • Coordinates treatment plans for routine services such as pre-exposure prophylaxis (PrEP), drug detoxification, and STI management;
  • Facilitates follow-up care in accordance with treatment plans established by the Primary Care Provider and interdisciplinary team;
  • Evaluates patient medication adherence as per the treatment plan, ensuring thorough medication reconciliation;
  • Performs other job-related duties as assigned.
