Position Overview

The Inpatient Care Management Supervisor will oversee and guide the entire Care Management team, which consists of RN Care Managers, Lead CM RN, CM Coordinators, Discharge Planners, Care Coordinators, Inpatient CM Supervisor, Complex CM Team, CM Educator, and UM CM Staff. This role may also involve supervising other management within the Care Management Department.

Key Responsibilities

  1. Supervise the assessment of patients for appropriate care levels, admission appropriateness, transfers, discharge assistance, and follow-up care in collaboration with the Emergency Department and Social Work care management team.
  2. Implement and review Care Management policies as necessary, ensuring compliance and effectiveness in service delivery.
  3. Provide clinical supervision for scheduling, payroll management, coaching, and educational needs to uphold Care Management policies and processes.
  4. Oversee the electronic referral system and documentation through the healthcare EMR, collaborating with Clinical Informatics to enhance documentation practices.
  5. Manage relationships with post-acute providers and community service providers, ensuring compliance with policies and plans related to vendor networks.
  6. Identify opportunities to enhance post-discharge services, aiming to reduce readmissions and improve community health outcomes.
  7. Collaborate with other managers within the Population Health Program to support the overall Care Management Department.

Essential Qualifications

  • Bachelor's degree in nursing is required; a Master's degree in nursing or a related field is preferred.
  • A minimum of five years of experience in acute care hospital settings, health plans, or ancillary services is required.
  • Prior supervisory or management experience in a clinical role is essential.
  • Current RN license is required; certification in Case Management or similar is expected within two years of hire.
  • Basic Life Support (BLS) certification is necessary.

Skills and Knowledge

  • In-depth knowledge of healthcare provider organizations, including acute and post-acute care facilities.
  • Familiarity with health plans and typical utilization management and medical management functions.
