Position Overview

The Inpatient Care Management Supervisor will oversee and guide the entire Care Management team, which includes RN Care Managers, Lead CM RN, CM Coordinators, Discharge Planners, Care Coordinators, and other essential roles. This position is pivotal in ensuring that all care management services are executed effectively, including patient assessments for appropriate levels of care, discharge planning, and follow-up services in collaboration with the Emergency Department and Social Work teams.

Key Responsibilities

  1. Supervise the Care Management team, ensuring compliance with established policies and procedures related to patient assessments, transfers, and discharge planning.
  2. Collaborate with the Emergency Department and Social Work Services to enhance the Care Management Program and support the broader Population Health initiatives.
  3. Oversee compliance with discharge planning policies and maintain relationships with post-acute care networks, including skilled nursing facilities and home health services.
  4. Identify and evaluate potential post-acute service providers to meet evolving clinical and social needs.
  5. Participate in multidisciplinary team meetings and hospital committees to promote effective care management practices.
  6. Foster a culture of professionalism and teamwork within the Care Management team, encouraging flexibility and priority setting.
  7. Ensure accountability among clinical and non-clinical staff for patient assessments and discharge planning.
  8. Manage the hiring process and assess staffing needs to align with population health requirements.
  9. Oversee payroll and attendance management, ensuring adherence to HR policies.
  10. Lead various clinical and operational projects aimed at reducing readmissions and enhancing transportation services.
  11. Work collaboratively with care coordinators and clinical staff to ensure seamless transitions for patients moving from inpatient to post-acute care.
  12. Establish and monitor key performance metrics related to quality, outcomes, and staff productivity.
  13. Design and implement training programs for staff regarding case management services and oversee clinical education initiatives.
  14. Address customer concerns and grievances systematically to improve service delivery.
  15. Coordinate with outpatient clinic staff to ensure smooth transitions for patients returning to community healthcare.
  16. Collaborate with other leadership roles to ensure adequate staffing and scheduling for transitional care functions.

Qualifications

  • Bachelor's degree in nursing is required; a Master's degree in nursing or a related field is preferred.
  • A minimum of five years of experience in acute care hospital settings or related services is required.
  • Prior supervisory or management experience in a clinical environment is essential.
  • Current RN license is required; certification in case management is expected within two years of hire.
  • Basic Life Support (BLS) certification is mandatory.
  • Experience with Medicaid/Medi-Cal patients and familiarity with government programs is preferred.
  • Knowledge of health care provider organizations and medical management functions is crucial.
