Position Overview

The Inpatient Care Management Supervisor will lead and oversee a comprehensive Care Management team, which includes RN Care Managers, Lead CM RN, CM Coordinators, Discharge Planners, Care Coordinators, Inpatient CM Supervisor, Complex CM Team, CM Educator, and UM CM Staff. This role is pivotal in ensuring effective patient assessments for appropriate care levels, admissions, transfers, and discharge processes, in collaboration with the Emergency Department and Social Work teams.

Key Responsibilities

  1. Supervise and manage the Inpatient Care Management team to ensure compliance with departmental policies and procedures, focusing on patient assessments, transfers, discharge needs, and follow-up care.
  2. Collaborate with other managers within the Population Health Program to enhance the Care Management initiatives and support the overall healthcare strategy.
  3. Oversee compliance with discharge planning policies and maintain relationships with post-acute care networks, including skilled nursing facilities and home health services.
  4. Evaluate and identify the need for additional provider networks to address evolving clinical and social needs.
  5. Participate in multidisciplinary conferences and hospital committees to foster teamwork and improve care delivery.
  6. Model leadership and professionalism, guiding staff in prioritization, teamwork, and adaptability in care management.
  7. Ensure accountability among clinical and non-clinical staff for conducting assessments, documenting discharge plans, and facilitating safe patient transitions.
  8. Manage staffing processes, including hiring and identifying skill mix requirements to meet patient population needs.
  9. Oversee payroll and attendance monitoring in compliance with HR policies.
  10. Lead various clinical and operational projects aimed at reducing readmissions and enhancing service delivery.
  11. Work closely with care coordinators and clinical staff to ensure seamless transitions from inpatient to post-acute care providers.
  12. Establish and monitor key performance metrics related to quality, outcomes, and staff productivity, providing regular reports to senior management.
  13. Design and implement training programs for staff and providers on post-acute care services and oversee the clinical education initiatives.
  14. Continuously evaluate and address customer concerns to enhance service quality.
  15. Coordinate services with community health departments to ensure comprehensive care for patients.
  16. Collaborate with other leadership teams to ensure adequate staffing and scheduling for transitional care functions.

Qualifications

  • Bachelor's degree in nursing is required; a Master's degree in nursing or a related field is preferred.
  • A minimum of five years of experience in acute care settings, health plans, or ancillary services is required.
  • Prior supervisory or management experience in a clinical environment is essential.
  • Current RN license is required; CCM or ACM certification must be obtained within two years of hire.
  • Basic Life Support (BLS) certification is mandatory.
  • Experience with Medicaid/Medi-Cal patients and government programs is preferred.
  • Knowledge of health care provider organizations and health plans is crucial.
