Position Overview
The Inpatient Care Management Supervisor will oversee and guide the entire Care Management team, which consists of RN Care Managers, Lead CM RN, CM Coordinators, Discharge Planners, Care Coordinators, and other clinical staff. This role is crucial in ensuring the effective assessment of patients for appropriate care levels, admissions, transfers, and discharge planning in collaboration with the Emergency Department and Social Work teams.

Key Responsibilities

  1. Supervise the Inpatient Care Management team, ensuring compliance with departmental policies and procedures related to patient assessments, transfers, and discharge planning.
  2. Collaborate with the Emergency Department Care Management and Social Work teams to enhance the overall Care Management Program.
  3. Oversee compliance with discharge planning policies and manage agreements with post-acute care networks.
  4. Assess the need for additional provider networks based on evolving clinical and social needs.
  5. Identify and evaluate potential post-acute service providers.
  6. Participate in multidisciplinary conferences and departmental meetings to promote effective communication and collaboration.
  7. Model leadership and professionalism, fostering teamwork and adaptability among staff.
  8. Ensure accountability for the performance of clinical and non-clinical staff in executing discharge plans and assessments.
  9. Manage the recruitment process and recommend staffing needs to meet the demands of the patient population.
  10. Oversee payroll management and monitor attendance in accordance with HR policies.
  11. Lead various clinical and operational projects aimed at reducing readmissions and enhancing patient care services.
  12. Work closely with clinical staff to ensure seamless transitions of care from hospital to post-acute providers.
  13. Establish and track management metrics related to quality, outcomes, and productivity.
  14. Design and implement training programs for staff and providers on post-acute care services.
  15. Continuously evaluate and address customer concerns and grievances.
  16. Coordinate services with community health organizations to ensure comprehensive care for patients.
Qualifications
  • Bachelor's degree in nursing is required; a Master's degree is preferred.
  • A minimum of five years of experience in acute care settings is required.
  • Prior supervisory or management experience in a clinical environment is essential.
  • Current RN license is required; certification in case management is expected within two years of hire.
  • Basic Life Support (BLS) certification is mandatory.
  • Experience with Medicaid/Medi-Cal patients is preferred.
Skills and Knowledge
  • In-depth knowledge of healthcare provider organizations and post-acute care services.
  • Familiarity with health plans and utilization management functions.
