Position Overview
The Inpatient Care Management Supervisor will oversee and lead the entire Care Management team, which comprises RN Care Managers, Lead CM RN, CM Coordinators, Discharge Planners, Care Coordinators, Inpatient CM Supervisor, Complex CM Team, CM Educator, and UM CM Staff. This role may also entail supervising other management within the Care Management Department. The responsibilities include evaluating patients for appropriate care levels, admission or observation suitability, HLOC transfers, discharge support, and follow-up care in collaboration with the Emergency Department and Social Work care management teams. The Manager will initiate, review, and implement Care Management policies as necessary.

Key Responsibilities
1. Supervise and manage the Inpatient Care Management team to ensure compliance with departmental policies and procedures, including assessments, HLOC transfers, discharge planning, and follow-up care.
2. Collaborate with the Manager of Emergency Department Care Management and the Director or Supervisor of Social Work Services to enhance the Care Management Program and support the Population Health Program.
3. Ensure adherence to compliance policies related to discharge planning and manage agreements with post-acute networks, including skilled nursing facilities and home health services.
4. Assess the need for additional provider networks in response to evolving clinical and social needs, working closely with management and the post-acute coordinator.
5. Participate in hospital committees and multidisciplinary conferences to contribute to the Care Management department's objectives.
6. Model leadership and professionalism, fostering teamwork and adaptability among staff.
7. Hold both clinical and non-clinical staff accountable for conducting initial assessments, documenting discharge plans, and collaborating effectively to ensure safe patient transitions.
8. Oversee the hiring process and recommend staffing needs to meet the population's requirements.
9. Manage payroll and monitor attendance in accordance with HR policies.
10. Lead various clinical, operational, and business projects aimed at reducing readmissions and enhancing transportation services.
11. Collaborate with ED Care Coordinators, Case Managers, Social Work, and clinical staff to ensure seamless patient transitions to post-acute providers.
12. Establish and monitor performance metrics related to quality, outcomes, productivity, and financial aspects, providing regular reports to senior management.
13. Design and implement training programs for staff and providers regarding post-acute services and case management.
14. Address internal and external customer concerns and grievances systematically.
15. Coordinate services with the Department of Health Outpatient Clinic staff to ensure comprehensive care for patients.

Qualifications
A. Education
- Bachelor’s degree in nursing required; Master’s degree preferred.
B. Experience
- Minimum of five years in acute care hospital settings or related services required.
- Previous supervisory or management experience in a clinical environment is essential.
- RN license required; CCM or ACM certification must be obtained within two years of hire.
- Basic Life Support (BLS) certification is mandatory.
C. Skills and Knowledge
- Extensive knowledge of healthcare provider organizations, including acute and post-acute care facilities.
- Familiarity with health plans and typical utilization management functions is beneficial.
