Job Summary

The Bilingual HUB Navigator will play a vital role in helping individuals navigate and access evidence-based community health promotion programs (EBPs) and services. This position requires a strong understanding of community health needs and the ability to provide information, support, and advocacy to individuals and communities.

Key Responsibilities
  • Process incoming referrals for EBPs and assess health status, risk, and social determinants of health.
  • Coach and empower individuals to take an active role in healthy lifestyles and educate them on available EBPs and community resources.
  • Register individuals into EBPs and assist with completion of forms and surveys.
  • Make additional referrals to social care services and monitor individual's enrollment and retention in referred programs and services.
  • Collect and manage individual's data, including pre and post surveys, to identify and report individual and community health needs.
  • Communicate with other service providers and referral sources to enhance care coordination.
Requirements
  • Must be 18 years old and have a high school diploma or GED.
  • Community Health Worker training preferred, with experience working with underserved and diverse populations.
  • Ability to speak and write fluent Spanish and English, with excellent customer service skills.
  • Child and Dependent Adult Abuse Mandatory Reporter Training is highly encouraged.
Work Environment

The physical demands of this job include sitting for up to 90% of the time, with occasional travel to health fairs and community events. The ability to carry a laptop and small supplies is required, as well as the ability to perform hand and wrist movements for administrative tasks.
