Job Summary

A Community Health Worker (CHW) is a vital member of our interdisciplinary care management team, serving as a trusted liaison between patients and healthcare providers. This role requires a unique blend of community knowledge, interpersonal skills, and healthcare expertise to facilitate seamless care coordination and patient empowerment.

Key Responsibilities
  • Screen patients for social influencers of health (SIoH) and connect them with relevant community resources and services.
  • Utilize motivational interviewing techniques to assess patients' non-clinical needs, values, and preferences, and develop personalized care plans.
  • Partner with patients to set goals and track progress, ensuring timely follow-up and documentation.
  • Facilitate communication and coordination between providers and patients, promoting effective care delivery and patient satisfaction.
  • Stay up-to-date on community services and resources, maintaining frequent contact with local agencies serving vulnerable populations.
Requirements
  • High school diploma or equivalent required; minimum 3 years of experience in healthcare, community involvement, or social services.
  • Strong understanding of community health principles, social determinants of health, and healthcare systems.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines.
