Job Summary

We are seeking a skilled Behavioral Health Navigator to join our team at UnityPoint Health. As a Behavioral Health Navigator, you will play a critical role in helping individuals navigate the mental health care system and achieve their goals.

Key Responsibilities
  • Provide exceptional customer service to clients and their families, ensuring a positive and supportive experience.
  • Work collaboratively with clients, healthcare providers, and community resources to develop and implement personalized care plans.
  • Conduct thorough assessments to identify client needs and develop strategies to address them.
  • Facilitate referrals to community resources and services, ensuring seamless transitions and continuity of care.
  • Provide ongoing support and follow-up services to clients, including phone contact and in-person visits.
  • Collaborate with healthcare providers to ensure timely access to needed care and services.
  • Document client interactions, progress, and outcomes in a timely and accurate manner.
Requirements
  • Bachelor's degree in a Human Services-related field.
  • 1 year of experience in mental health-related field.
  • Valid driver's license when driving any vehicle for work-related reasons.
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Knowledge of mental health principles, practices, and resources.
