As a vital member of our organization, the Patient Care Coordinator plays a crucial role in achieving our mission. This position is essential in the development and execution of a centralized pharmacy system that caters to the needs of our community health centers and affiliated clinics. The Patient Care Coordinator is tasked with overseeing patient enrollment and re-enrollment in various pharmacy assistance initiatives.

Key responsibilities include:

  • Utilizing computer systems to input data, track medication processes, and monitor patient participation in assistance programs.
  • Acting as a liaison between patients, their families, and pharmacy personnel.
  • Providing interpretation and translation services as necessary.
  • Delivering educational resources to patients concerning their medications, consultations, and follow-up care while upholding patient confidentiality and rights.
  • Supporting the Medication Therapy Management (MTM) pharmacy and engaging in specialty pharmacy program initiatives.
  • Offering administrative assistance within the pharmacy when required.

This is a full-time role, typically requiring 40 hours per week, with a standard schedule of Monday through Friday.

Qualifications:

  • High School diploma or equivalent is mandatory.
  • Current Pharmacy Technician licensure in Nebraska is essential.
  • National Pharmacy Technician Certification must be obtained within the timeframe specified by Nebraska Pharmacy Laws.
  • Proficiency in Microsoft Office, especially Excel, is required.
  • Fluency in both English and Spanish is necessary.
  • Strong oral and written communication skills, along with excellent organizational abilities, are essential.
  • Ability to think critically and resourcefully when addressing challenges.
  • Demonstrated mathematical skills, including basic operations and analytical problem-solving.
  • Knowledge of medication classifications and their applications.
  • Awareness of assistance programs and community resources tailored for low-income and medically underserved populations.
  • Experience working collaboratively with diverse groups in a coordinating capacity.
