We are currently seeking a dedicated Patient Care Coordinator to enhance our team. The Patient Care Coordinator (PCCO) plays a vital role in our mission by facilitating the development and execution of a centralized pharmacy system that supports both OneWorld Community Health Centers and the Network Clinic. The PCCO is tasked with managing patient enrollment and re-enrollment in pharmacy assistance initiatives.

Key Responsibilities:

  • Utilize computer systems to input data, oversee medication processes, and track patient participation in assistance programs.
  • Act as a liaison between patients, their families, and pharmacy personnel, ensuring clear communication and support.
  • Provide interpretation and translation services as necessary, ensuring that all patients receive the information they need.
  • Deliver educational resources to patients regarding their medications, consultations, and follow-up care, while upholding patient confidentiality and rights.
  • Support the Medication Therapy Management (MTM) pharmacy and contribute to specialty pharmacy program teams.
  • Offer administrative assistance within the pharmacy as required.

This is a full-time opportunity with a standard schedule of 40 hours per week, typically Monday through Friday.

Qualifications:

  • High School diploma or equivalent is mandatory.
  • Current Pharmacy Technician licensure in Nebraska is essential.
  • National Pharmacy Technician Certification must be obtained within the timeframe set by Nebraska Pharmacy Laws.
  • Proficiency in Microsoft Office, especially Excel, is required.
  • Fluency in both English and Spanish is necessary.
  • Strong verbal and written communication skills, along with excellent organizational abilities, are essential.
  • Ability to think critically and resourcefully troubleshoot issues.
  • Mathematical proficiency, including basic operations and analytical problem-solving skills.
  • Understanding of medication classifications and their applications.
  • Awareness of assistance programs and community resources for low-income and medically underserved populations.
  • Experience working with diverse groups in a coordinating capacity.
