Job Overview

Position Title: Resource Coordinator

Reports To: Program Director

Program Summary:

Our mission is to assist individuals in transitioning from temporary shelter to lasting self-sufficiency through a strength-based approach to case management. The FaSST/Connections unit focuses on behavioral health case management, offering evaluation, linkage, and coordination of essential services for individuals and families residing in various shelters.

Role Summary:

  • We seek dedicated, compassionate individuals who aspire to positively impact the lives of those facing homelessness.
  • Resource Coordinators will engage with individuals and families who are experiencing homelessness and may have mental health or substance use challenges, providing advocacy, ongoing support, and empowerment.
  • This role involves collaborating with participants and their support networks to identify and overcome barriers, helping them achieve their personal goals, which may include accessing mental health services, resolving legal issues, securing income, and connecting to educational opportunities.

Key Responsibilities:

  • Complete all required documentation, including service documents and personal goal plans that emphasize the strengths and needs of participants.
  • Assess and evaluate participant needs, providing connections to appropriate services.
  • Monitor service delivery through regular contact with participants and involved service providers.
  • Collaborate with participants to utilize their individualized strength-based personal goal plans, reviewing and adjusting as necessary.
  • Conduct regular home visits and accompany participants to medical appointments and treatment facilities as needed.
  • Educate and advocate for participants regarding benefits, treatment options, self-advocacy, and natural support systems.

Required Skills:

  • Demonstrated understanding and respect for each individual's unique recovery journey, providing trauma-informed, strength-based support.
  • Exhibit professionalism and ethical service consistently.
  • Able to work independently and collaboratively within a team.
  • Strong time management skills and adaptability.
  • Excellent verbal and written communication abilities.
  • Proficient in computer use, including electronic health records, video conferencing, and email.
  • Familiarity with navigating the city via car or public transportation.

Qualifications:

  • Bachelor's degree in a Human Services field is required.
