Key Responsibilities- Conducts and completes annual evaluations to identify individual goals, preferences, and capabilities, ensuring clear communication throughout the assessment process.
- Identifies community support needs and collaborates with the individual's support network to establish connections with relevant agencies.
- Facilitates the development of social connections and relationships by teaching essential skills and creating opportunities that leverage individual strengths.
- Ensures all team members are aware of upcoming Individual Support Plan (ISP) meetings.
- Assists individuals in recognizing advocacy needs by gathering and analyzing relevant information.
- Explores vocational and educational interests with individuals, providing support for job readiness and ongoing career advancement.
- Implements and monitors personalized plans, reporting on progress towards specific outcomes aligned with individual preferences.
- Educates team members and community stakeholders about individualized plans while maintaining confidentiality and ethical standards.
- Actively engages in reviewing individual outcomes, maintaining accurate documentation, and submitting reports promptly.
- Communicates findings and recommendations clearly to individuals, ensuring follow-up and necessary adjustments.
- Acts as a liaison for Supports Coordinators, coordinating meetings and communicating any necessary plan modifications.
- Collects and analyzes data from monthly and quarterly reports, providing feedback to team members through various communication channels.
- Strong verbal and written communication skills.
- Excellent organizational abilities with the capacity to manage a diverse caseload.
- Ability to engage effectively with individuals in various environments.
- Demonstrated patience and adaptability.
- Proficiency in MS Office and other Windows-based applications.
- Bachelor's Degree in Psychology, Special Education, or a related field with a minimum of 2 years of experience working with individuals with intellectual disabilities (preferred).
- Associate's Degree in Psychology, Special Education, or a related field, or at least 60 college credits with 4 years of relevant experience.
- Valid driver's license, personal vehicle, and insurance are required.
