Job Summary
Project H.O.M.E. is seeking a Case Manager and Community Outreach Specialist to join our team. In this role, you will provide a continuum of services for children, youth, and adults experiencing homelessness, including street outreach, supportive, transitional, and permanent housing options, community-based supportive employment and education opportunities, and more.
Key Responsibilities
- Provide street outreach services to individuals and families in need
- Assist with the development and implementation of supportive, transitional, and permanent housing options
- Collaborate with community-based organizations to provide supportive employment and education opportunities
- Conduct regular check-ins with clients to assess their needs and provide support
Requirements
- College graduate
- English language proficiency
- Ability to work in a fast-paced environment and prioritize multiple tasks
