Position Overview: We are looking for a Post-Acute Care Coordinator to join our team, ensuring the delivery of high-quality, efficient care to all patients.

Job Summary and Qualifications: The Post-Acute Care Coordinator will serve as a liaison between internal and external referral sources and CarePartners' Post-Acute Service Lines. This role is pivotal in building relationships, providing education, and fostering collaboration with hospitals, skilled nursing facilities, assisted living facilities, ambulatory and inpatient care managers, physicians, payors, and other community referral sources.

The Coordinator will facilitate appropriate patient placements through screenings and assessments in partnership with SNF Discharge Planners, Inter-Disciplinary Teams, Ambulatory Care Managers, and other healthcare professionals.

Key responsibilities include:

  • Collaborating with Post-Acute providers, SNFs, and physician practices to ensure a seamless referral and admission process.
  • Working closely with health system and community colleagues to promote quality-driven, best practices across the care continuum.
  • Maximizing positive financial outcomes for designated patient populations by facilitating appropriate Post-Acute placements and engaging in Goals of Care/Advanced Care Planning discussions.
  • Implementing strategies to prevent readmissions and lower total care costs through effective utilization of Post-Acute services.

Qualifications:

Required Education: Bachelor’s degree in Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Work, Psychology, Business, Health Sciences, or a related field.

Preferred Education: Master’s degree in Nursing or a related field.

Required Licenses/Certifications: If a Registered Nurse, must hold and maintain current licensure with the appropriate nursing board. Additional certifications in Case Management may be required.

Required Experience: A minimum of two years of recent experience in a health-related field.

Preferred Experience: Eight years of recent clinical experience in a relevant practice area.
