We are seeking a committed Physical Therapist for CarePartners Home Health to join our dedicated team.

Job Summary and Qualifications

Job Summary:


The Primary Physical Therapist in Home Health oversees an assigned caseload of patients, delivering therapy and clinical case management, taking responsibility for enhancing patient outcomes while managing resources to ensure cost-effective, clinically effective, and coordinated services.

Conducts thorough assessments/examinations/evaluations, develops/modifies care plans in collaboration with patients/caregivers and physicians, provides treatment visits, and facilitates discharge planning to engage patients/caregivers in achieving patient goals, clinical outcomes, and positive patient experiences.

The Primary Physical Therapist delivers the majority of the patient's physical therapy and communicates essential information/coordinates with PTAs and other care team members to support patient care throughout home health and the continuum of care.


Job Qualifications:

Required Education:
Bachelor of Science degree in Physical Therapy.

Required License:
Current North Carolina license to practice Physical Therapy; Driver's license required. BLS/CPR Certification.

Required Experience:
One to two years of relevant experience.

Preferred Experience:

Clinical affiliations with a diverse range of acute care procedures, particularly in orthopedics, neurology, cardiopulmonary, and care of burns/open wounds are advantageous.

Basic Cardiac Life Support must be obtained within 7 days of employment start date.
Licensed Physical Therapist.
Driver's License.
Bachelor's Degree.
