Position Overview

As a vital member of the healthcare team, the Patient Navigator plays a crucial role in facilitating access to essential health services for clients. This position requires a compassionate individual dedicated to supporting patients in navigating their healthcare journey.

Key Responsibilities

  • Assist clients in utilizing available health resources effectively.
  • Conduct initial assessments to identify client needs and ensure appropriate referrals to health services.
  • Provide informal counseling and educational support to clients, helping them understand their options.
  • Support clients in scheduling medical appointments and accompany them when necessary.
  • Maintain accurate and comprehensive client records, documenting all interactions and services provided.
  • Foster strong relationships with external referral sources and internal team members to enhance service delivery.

Qualifications

Education:

  • High School diploma or equivalent with two years of relevant experience, or an Associate's/Bachelor's degree, or four years of applicable experience.
