Position Overview
As a vital member of the healthcare team, the Patient Navigator plays a crucial role in facilitating access to essential health services for clients. This position is dedicated to ensuring that individuals receive the support they need to navigate the healthcare system effectively.
Key Responsibilities
- Assist clients in utilizing available resources to access health-related services.
- Conduct initial assessments to identify client needs and maintain ongoing follow-up to ensure appropriate guidance.
- Provide informal counseling, education, and social support to clients, including assistance with scheduling medical appointments and completing necessary applications.
- Foster and maintain strong relationships with both external referral sources and internal team members.
- Document all client interactions and services provided to maintain accurate records for future reference.
Qualifications
Education:
- High School diploma or equivalent with two years of relevant experience, or an Associate's/Bachelor's degree, or four years of applicable job-specific experience.
