Job Summary:

An RN Care Team Coordinator works collaboratively with the interdisciplinary team to develop and implement individualized plans of care for assigned patients. This role requires a strong understanding of Conditions of Participation and Joint Commission Standards to ensure regulatory compliance.

Key Responsibilities:
  • Develop and implement individualized plans of care, including measurable patient-centered goals, in collaboration with the interdisciplinary team.
  • Collaborate and participate in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings to ensure coordination of services.
  • Participate in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
  • Precept new staff to support professional relationships with newly hired team members as required.
Requirements:
  • Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years acute care nursing experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
