Position Overview

Key Information Regarding the LPN Care Team Coordinator Role:

  • Full-Time position, Monday through Friday, operating hours from 8:00 AM to 4:30 PM.
  • Collaborates with an interdisciplinary team to develop tailored, comprehensive care plans for designated patients.

Required Qualifications:

  • Associate degree in a healthcare-related discipline is mandatory; an active Licensed Practical Nurse license is preferred.
  • A minimum of two years' experience in a healthcare environment and a solid understanding of person-centered care principles are essential.
  • Experience with Quality Improvement Initiatives is highly desirable.
  • Capability to design Quality Improvement activities as necessary.
  • Prior customer service experience or related background is preferred.
  • Strong clerical, organizational, and computer skills, including proficiency in Word, Excel, and PowerPoint, along with excellent verbal and written communication abilities.
  • Must possess the ability to multitask effectively and work collaboratively within an interdisciplinary framework to adhere to care plans.
  • This role will primarily be home-based but may require occasional travel for meetings and patient visits.
  • CPR certification is preferred and will be required during orientation upon hire.
  • Notary designation or intention to obtain within six months of employment.
  • A valid Driver's License with a clean driving record is required, along with successful completion of a driving record check.
  • Must be able to drive during various conditions, including daytime, nighttime, and inclement weather, as travel to multiple locations within the service area is necessary.
  • State minimum automobile insurance coverage is required.
  • Successful completion of a criminal background check is mandatory.
