Job Summary

An RN Care Team Coordinator plays a vital role in creating comprehensive individualized plans of care for assigned patients within an interdisciplinary team.

Key Responsibilities
  • Develops and implements individualized plans of care, including measurable patient-centered goals, in collaboration with the interdisciplinary team.
  • Collaborates and participates in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings to ensure coordination of services.
  • Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
  • Precepts new staff to support professional relationships with newly hired team members as required.
Requirements
  • Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years acute care nursing experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  • CPR Certified.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
