Nursing Care Coordinator

An RN Care Team Coordinator plays a vital role in creating comprehensive individualized plans of care for assigned patients within an interdisciplinary team.
Key Responsibilities- Develops and implements individualized plans of care, including measurable patient-centered goals, in collaboration with the interdisciplinary team.
- Collaborates and participates in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings to ensure coordination of services.
- Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
- Precepts new staff to support professional relationships with newly hired team members as required.
- Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
- Active Registered Nurse license in the State of Ohio without any board actions.
- Two years acute care nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
- CPR Certified.
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime, or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance Coverage.
