Job Summary

We are seeking a skilled and compassionate Patient Care Coordinator to join our team at Wooster Community Hospital. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Main Responsibilities:

  • Assist nurses in meeting patient needs as directed by registered nurses and licensed practical nurses.
  • Communicate effectively with patients, family members, and other healthcare professionals to ensure seamless care delivery.
  • Follow established nursing philosophy and practice models, including the Synergy model and Service Standards outlined in the Employee Handbook.
  • Collaborate with department directors, clinical managers, charge nurses, and other healthcare team members to ensure high-quality patient care.

Requirements:

  • High school diploma or equivalent required.
  • Must successfully complete Wooster Community Hospital's PCA training program upon hire.
  • EMT or STNA certification may be required for certain positions.
  • No written disciplinary action in the last 12 months.
  • Maintain current AHA Healthcare Provider BLS certification.

Preferred Attributes:

  • Personal computer experience and keyboarding skills.
  • Medical terminology course or demonstrated knowledge.
  • Successful completion of clinical experience in LPN or RN program or STNA certification.

Work Environment:

  • Exposed to blood, body fluids, and airborne pathogens; personal protective equipment required.
  • Must be able to push, pull, lift, carry, turn, crouch, and bend as necessary to complete nursing care tasks.
  • May be required to take on-call status.
  • Reassignment to another unit to perform duties within current job description.
  • Subject to STHR (short-term hour reduction) according to policy.

Work Schedule:

Full-time night shift position working three 12-hour shifts per week (7am-7pm) with the option of every third weekend.
