Position Summary for Registered Nurse Case Manager (RNCM):

Positions are available for Registered Nurse Case Managers (RNCM) in various counties. Responsibilities include:

  • Planning, organizing, and directing home health care services, including initial evaluations, pain management, and care plan development.
  • Ensuring a high-quality patient experience and positive health outcomes through personalized care plans, with proactive adjustments based on patient needs.
  • Preparing thorough and timely clinical documentation, coordinating care with an interdisciplinary team, and collaborating with primary care physicians for continuity of care.
  • Providing education and coaching to patients and caregivers to empower self-management and improve quality of life.

Minimum Qualifications for Registered Nurse Case Manager (RNCM):

Candidates must possess:

  • A current CA Registered Nurse (RN) license
  • A valid CPR card for Healthcare Providers
  • At least one year of RN experience in Home Healthcare (preferred)

Additionally, candidates should have a valid driver's license and an insured vehicle in good working condition. Strong observation, problem-solving, and communication skills are essential, along with the ability to work independently.
