POSITION SUMMARY: The Recovery Coach (RC) is a vital component of the Moms Do Care initiative, dedicated to assisting individuals facing challenges related to substance use. This role focuses on providing peer recovery support services to pregnant, post-partum, and parenting individuals (PPPP) experiencing substance use disorders. Candidates must have personal experience as a parenting individual in recovery. The RC aids PPPP in realizing their health and wellness objectives through a supportive and understanding approach. Utilizing harm reduction strategies, the RC can assist both those pursuing sustained recovery and those opting for safer substance use practices. A comprehensive understanding of successful recovery pathways, the ability to convey personalized health messages, and the capacity to connect patients with BNHC and community resources are essential. This role requires strong organizational and communication skills, along with the ability to work autonomously and collaboratively within a team. The RC will report to the Moms Do Care Program Supervisor and receive supervision from BNHC's Peer Support Supervisor. Work settings may include various outreach locations and community programs.
KEY RESPONSIBILITIES:
  1. Advocate for the Moms Do Care program within the community to enhance patient access, including outreach to local healthcare providers and treatment facilities.
  2. Establish and nurture professional relationships with potential and current BNHC patients to effectively deliver peer recovery support services.
  3. Collaborate with Behavioral Health Clinicians to facilitate support groups designed for pregnant, post-partum, and parenting individuals.
  4. Create and execute wellness and relapse prevention strategies with patients.
  5. Implement harm reduction techniques, including overdose prevention education and access to Narcan (naloxone) and other resources.
  6. Provide relevant recovery information, referrals, and linkages to assist individuals in improving their recovery and minimizing risky behaviors.
  7. Offer ongoing support and follow-up to promote recovery, health, and overall wellbeing, acknowledging various recovery pathways.
  8. Maintain open communication with community organizations and healthcare providers to enhance resource accessibility.
  9. Utilize non-judgmental language and motivational interviewing techniques to help patients identify and prioritize their needs.
  10. Gather and disseminate reliable information on health topics pertinent to the served population.
  11. Facilitate warm handoffs to the Moms Do Care program.
  12. Document patient interactions in the electronic medical record system.
  13. Perform clerical tasks such as managing phone communications and appointment scheduling.
  14. Participate in required departmental meetings and adhere to patient privacy regulations.
  15. Other responsibilities as assigned.
SCOPE OF PRACTICE: The following activities are within the recovery coach's scope:
  • Motivate patients and celebrate their achievements.
  • Provide constructive feedback and suggestions to help patients meet their recovery goals.
  • Serve as a role model and mentor, demonstrating healthy living practices.
  • Assist in identifying and resolving challenges in a supportive manner.
  • Act as a liaison to the recovery community and treatment services.
  • Advocate for patients' needs.
  • Employ motivational interviewing to aid in developing wellness and relapse prevention plans.
  • Complete wellness and relapse prevention plans with program participants.
MINIMUM QUALIFICATIONS:
  • High school diploma or GED required.
  • Proficiency in Microsoft Office and the ability to research community resources independently.
  • Familiarity with community, social, and health resources, particularly those related to substance use.
  • Understanding of recovery principles and lived experience with substance use disorder and motherhood.
  • Experience with electronic health records is advantageous.
  • Ability to apply cultural competency and equity principles in patient care.
CERTIFICATION REQUIREMENTS:
  • Certified Addictions Recovery Coach (CARC) or completion of Recovery Coach Academy with a commitment to certification.
WORKING CONDITIONS: The role is based in a dynamic community health center located in an urban area. Physical demands include lifting light equipment (up to 20 lbs.) and may involve some walking. The environment is fast-paced with potential interruptions. Stress may arise from a high volume of work and diverse activities. Flexibility to work overtime may be required to address emergencies or project deadlines. Visual and auditory acuity is necessary for effective communication and documentation. Weekend or evening work may be necessary based on patient and community needs.
