Job Summary:

Key Responsibilities:

  • Develop and maintain meaningful relationships with patients, utilizing your personal experiences to provide a supportive and non-judgmental space for them to share their struggles and concerns.
  • Collaborate with our team to provide evidence-based treatment modalities, including diversion from emergency room care and hospitalizations.
  • Act as an advocate for patients, promoting self-care, illness self-management, and wellness and recovery.
  • Work closely with outside agencies, including hospitals, community-based agencies, and first responders, to ensure seamless care coordination.
  • Maintain accurate and timely documentation, participating in team meetings and contributing to the development of individualized recovery plans.

Requirements:

  • A high school diploma or equivalent is required; a relevant degree is preferred.
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.
  • Lived experience with mental illness, with the ability to meet the essential requirements of the job.
  • High level of professionalism and ethical behavior.
  • Ability to work sensitively with individuals from diverse backgrounds and cultures.
  • Effective communication skills, with the ability to work collaboratively with peers, managers, and support staff.
  • Maintain all documentation in a complete and timely manner, complying with all HIPAA requirements.
  • Certification as a Peer Specialist or Recovery Coach is preferred.
  • A valid driver's license and auto insurance.
