The Transitional Age Youth Support Specialist utilizes personal experiences related to mental health and substance use challenges to foster hope and encouragement in young adults facing similar issues. This role emphasizes building supportive relationships with Transitional Age Youth (TAY) and Young Adults (YA) through empathetic listening, recognizing their unique strengths, and enhancing their communication with family, friends, and support networks.

Program Overview: Flexible Support Services aims to assist children and youth diagnosed with serious emotional disturbances (SED) and their families. The program operates within a wraparound model, integrating family-driven services, natural supports, and professional teamwork.

The Specialist provides strength-based, peer-to-peer education, coaching, and guidance to TAY/YA across various environments, including home, educational settings, and community spaces.

Key Responsibilities:

  • Engage with new families in a welcoming and supportive manner.
  • Share personal recovery experiences to inspire and motivate TAY/YA.
  • Guide TAY/YA in effective communication with their support systems.
  • Assist youth in identifying their strengths through creative vision mapping techniques.
  • Collaborate with TAY/YA's support teams to ensure their aspirations guide treatment and life goals.
  • Model safe and effective sharing of personal experiences.
  • Promote self-care by demonstrating healthy habits and sharing beneficial resources.
  • Work with families, treatment providers, and other supports to ensure integrated and coherent service delivery.
  • Connect TAY/YA with peer support resources and community initiatives.
  • Encourage self-efficacy and empowerment among TAY/YA.
  • Participate in regular supervision and training sessions.
  • Maintain accurate documentation of progress and service delivery.
  • Perform additional duties as assigned in alignment with organizational policies.

Qualifications:

  • Personal experience with mental health and substance use recovery, coupled with training to share these experiences effectively.
  • Valid driver's license and reliable transportation are required.

Preferred Experience:

  • Familiarity with state systems such as DMH, DCF, DDS, and DYS is advantageous.

Skills and Abilities:

  • Ability to approach TAY/YA with an unconditional positive regard.
  • Flexibility to respond to youth in stressful or crisis situations.
  • Strong multitasking and organizational skills in a dynamic environment.
  • Proficient in administrative tasks, professional writing, and communication.
  • Willingness to travel as needed within the service area.
