Job Summary: We are seeking a highly motivated and compassionate Community Outreach Specialist to join our team at CN Guidance and Counseling Services. As a key member of our outreach team, you will play a vital role in connecting individuals with substance use disorders and their families with our comprehensive services.
Key Responsibilities:
- Provide outreach and engagement services to individuals and families affected by substance use disorders.
- Develop and maintain relationships with community partners, including healthcare providers, social service agencies, and other stakeholders.
- Assist with the coordination of care and services for individuals and families, including referrals to treatment programs and other community resources.
- Provide health coaching and recovery support to individuals and families, including education on medication-assisted treatment and other evidence-based practices.
- Participate in quality improvement initiatives and contribute to the development of new programs and services.
Requirements:
- High School Diploma or GED
- Certified Recovery Peer Advocate (CRPA) Certification
- Knowledge of medication-assisted treatment, opiate overdose prevention, and substance use disorders treatment and recovery services
- Relevant life experience, including personal experience living with a disability, history of substance use, or social/emotional challenge
- Valid New York State Drivers License
