As a Community Outreach Specialist, you will play a vital role in assisting individuals in their recovery journey by providing outreach, support, and coordination of care.
Key Responsibilities- Provide outreach to clients to facilitate their engagement in services and offer additional supports to ensure successful participation in Medication-Assisted Treatment and mobile services.
- Offer health coaching and recovery support, focusing on self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
- Provide transportation for and accompany clients to medical and/or other appointments to facilitate their engagement and follow-through as needed.
- Maintain accurate and timely records, statistics, and reports in accordance with policy and procedure.
- Coordinate care planning with other providers of services/resources to ensure goal-directed, collaborative care, including care transitions.
- Act as a resource/consultant to team members on issues related to substance abuse.
- Provide telephonic and face-to-face outreach, engagement, and service planning in the field.
- Link clients to community services, including medical, behavioral, residential, entitlement, and other needed services per interdisciplinary care plan.
- Monitor overall service delivery to clients to ensure coordination and continuity, advocating with service providers/resources as needed.
- Provide crisis intervention and follow-up services.
- High School Diploma or GED
- Certified Recovery Peer Advocate (CRPA) Certification (or provisional certification upon hire and receipt of full certification within one year)
- Knowledge of medication-assisted treatment, opiate overdose prevention, and substance use disorders treatment and recovery services
- Relevant life experience, including personal experience living with a disability, history of substance use, or social/emotional challenge
- Valid New York State Driver's License and clean DMV record
