Job Summary

We are seeking a compassionate and spiritually grounded individual to join our team as a Spiritual Care Coordinator at Empyrean Hospice. As a key member of our interdisciplinary team, you will provide spiritual care and support to patients and their families, ensuring a holistic approach to end-of-life care.

Key Responsibilities
  • Represent Empyrean Hospice with professionalism and compassion, upholding our core values of integrity and clinical excellence.
  • Conduct spiritual assessments of patients and families, identifying their unique needs and developing individualized plans of care.
  • Collaborate with the interdisciplinary team to provide comprehensive care, including consultation, education, and support.
  • Provide direct spiritual care to patients and families, including counseling, support, and guidance.
  • Recruit and develop community chaplains, spiritual counselors, and volunteers to meet patient and family needs.
  • Maintain accurate records of spiritual care services and activities.
  • Plan and coordinate memorial services, ensuring a dignified and respectful approach to end-of-life care.
Requirements
  • Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).
  • Experience working with death and dying, individuals, and families.
  • Self-directed with the ability to work with minimal supervision, yet as part of a collaborative care team.
  • Understanding of hospice care philosophy and grief response, with experience in counseling individuals and groups.
  • Comfortable in an interfaith setting, with the ability to accept diverse lifestyles, cultures, beliefs, and values.
  • Strong communication and organizational skills, with the ability to network with community clergy and congregations.
  • Licensed driver with a reliable vehicle, insured in accordance with state or organization requirements.
