Job Highlights:
- Manage a patient load of approximately 15 individuals per day.
- Full-time employment available, with options for part-time candidates.

Key Responsibilities:
- Deliver high-quality, safe, and cost-effective healthcare services in post-acute care settings.
- Collaborate with interdisciplinary teams to adhere to CMS guidelines.
- Work closely with hospice, palliative care, and home care agencies to ensure smooth patient transitions.
- Maintain timely documentation and billing through our EMR system, ensuring compliance with Medicare standards.
- Recent graduates are encouraged to apply, and we offer cross-training for qualified candidates. Experienced professionals are also welcome to join our team.
Compensation and Benefits:
We offer a competitive salary range of $105,000 to $115,000 annually, with opportunities for additional incentives. Full-time employees receive comprehensive health, dental, and medical benefits, along with 20 days of paid time off (PTO) and unlimited clinical education support tailored to post-acute care.
