About the Role

We are seeking a compassionate and spiritually grounded individual to join our team as a Hospice Spiritual Care Coordinator. As a member of our interdisciplinary team, you will provide spiritual care and support to patients and their families, ensuring a holistic approach to end-of-life care.

Key Responsibilities
  • Represent Empyrean Hospice with professionalism and compassion, upholding our core values of integrity and clinical excellence.
  • Conduct spiritual assessments of patients and families, identifying their unique needs and developing individualized plans of care.
  • Collaborate with the interdisciplinary team to provide education, consultation, and support on spiritual care.
  • Provide direct spiritual care to patients and families, including facilitating memorial services and supporting community chaplains and spiritual counselors.
  • Maintain accurate records of spiritual care services and activities.
  • Recruit and develop community contacts, including chaplains and spiritual counselors, to support patient and family needs.
Requirements
  • Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).
  • Experience working with death and dying, individuals, and families.
  • Self-directed with the ability to work with minimal supervision, yet as part of a comprehensive care team.
  • Understanding of hospice care philosophy and grief response, with experience in counseling individuals and groups regarding loss.
  • Comfort in an interfaith setting, with the ability to accept diverse lifestyles, cultures, beliefs, and values.
  • Strong communication and organizational skills, with the ability to network with community clergy and congregations.
  • Licensed driver with a reliable vehicle, insured in accordance with state or organization requirements.
