As a Medical Social Worker, you will play a vital role in providing patient-centered care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Assist patients and families with health-related financial, social, and emotional concerns, and provide effective coping skills.
  • Provide social work services in accordance with the plan of care, ensuring seamless communication with other healthcare team members and care center office staff.
  • Educate patients, family members, and team members on medical social work interventions and treatment plans.
  • Perform care planning, transfer, and discharge functions, identifying risks or threats to patient health and implementing appropriate interventions.
  • Communicate with physicians and other healthcare team members to coordinate care effectively.
Requirements
  • Holds a master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
  • Has one year of social work experience in a healthcare setting.
  • Meets state and federal requirements for this position.
  • Current CPR certification.
