Clinical Operations Nurse Supervisor

POSITION OVERVIEW:


The Clinical Operations Nurse Supervisor is a licensed registered nurse responsible for overseeing services at the medical respite facility dedicated to supporting homeless individuals.

This role involves assessing urgent health issues, coordinating care with community healthcare providers, and collaborating effectively with PHMC's Health Network.

All operations are conducted in partnership with direct care nursing staff, case management professionals, and behavioral health specialists.

The Nurse Supervisor will work closely with the Respite Program Administrator to design services and programs in alignment with National Medical Respite Standards.

Administrative oversight will be provided by the Program Administrator, while clinical guidance will come from the Director of Nursing within the PHMC Health Network.


KEY RESPONSIBILITIES:

  • Manage clinical and operational workflows within the Medical Respite facility.
  • Facilitate initial orientation, continuous training, and support for all team members involved in respite care.
  • Evaluate all referrals and determine suitability for admission into the respite program.
  • Coordinate care with various medical facilities, including emergency departments, skilled nursing facilities, and outpatient services.
  • Organize and lead weekly team meetings to assess patient progress and plan for discharges.
  • Collaborate with social services teams to ensure appropriate post-respite care for patients.
  • Ensure that services are centered around patient needs, incorporating self-management strategies.
  • Develop and promote outreach programs focused on preventive care and education on health topics such as HIV/AIDS, infection control, chronic disease management, and more.
  • Work alongside nursing staff to provide comprehensive nursing case management, including tracking referrals and ensuring patient adherence to follow-up care.
  • Create clinical protocols in partnership with the Director of Nursing and Medical Director.
  • Establish and implement protocols for medication storage and management.
  • Oversee inventory and procurement of nursing and medical supplies.
  • Adhere to HIPAA regulations, ensuring patient confidentiality and privacy.
  • Manage medication according to established guidelines set forth by medical respite standards.
  • Assess and treat patients in line with best practices for post-acute care in a medical respite environment.
  • Provide on-call support as necessary.
  • Maintain positive relationships with funding organizations and community partners.
  • Utilize clear and effective communication in both written and verbal forms, complying with departmental policies regarding patient care documentation.
  • Perform additional related tasks as assigned.
  • Act as a mandated reporter in accordance with state regulations and organizational policies.

SUPERVISORY DUTIES:

  • Ensure adequate staffing coverage around the clock for the facility.
  • Collaborate with the Program Administrator and Director of Nursing to develop and implement workflow processes.
  • Communicate urgent needs to the Program Administrator and Director of Nursing promptly.
  • Activate emergency response plans as required.
  • Assist in coordinating external audits and site visits from regulatory bodies.
  • Conduct performance evaluations for designated team members.

COMPLIANCE OBLIGATIONS:

  • Understand and comply with PHMC's compliance standards as outlined in the Code of Conduct and related policies.
  • Stay informed about relevant federal, state, and organizational regulations and policies.
  • Adhere to HIPAA and confidentiality standards applicable to the role.
  • Comply with standards set by the Department of Public Health, the Joint Commission, and other regulatory agencies.
  • Follow all PHMC Policies and Procedures diligently.
  • Learn and comply with Infection Control and Environment of Care Guidelines as part of ongoing education.

SKILLS REQUIRED:

  • Strong verbal and written communication skills in English.
  • Ability to effectively engage with individuals experiencing chronic homelessness.
  • Familiarity with electronic health records (EHR) or similar patient management systems.

EXPERIENCE REQUIRED:

  • Background in primary care, community health, or behavioral health is preferred.
  • Minimum of 3 years of nursing experience is required.
  • At least 2 years of leadership or supervisory experience is essential.

EDUCATION & LICENSURE:

  • A Bachelor's degree in nursing from an accredited institution is required.
  • Must hold a valid Registered Nurse (RN) license in the state of Pennsylvania.
