Job Overview

  • Collaborates with Physician Advisors for high-risk member consultations.

  • Works with the Quality Management Department to monitor provider stability and address any issues.
  • Maintains effective communication with other departments to ensure operational efficiency.
  • Ensures critical information regarding members and providers is communicated effectively.
  • Analyzes data related to member services and clinical operations to inform interventions.
  • Ensures that team members are knowledgeable about evidence-based practices and integrates these discussions into supervision.
  • Utilizes data-driven approaches for decision-making.
  • Models exceptional customer service in interactions with members and providers.
  • Handles on-call responsibilities as assigned.
  • Performs additional duties as required.

Team-Specific Duties:

  • Develops and manages clinical specialty programs.
  • Designs and enhances care management strategies for team growth.
  • Establishes partnerships with health providers to improve communication and collaboration.
  • Works with community organizations to ensure comprehensive, evidence-based care for members.
  • Focuses on targeted interventions to reduce readmissions and service usage.
  • Utilizes data to identify members with specific behavioral health needs and ensures they receive appropriate support.
  • Collaborates with the Integrated Care Plan team to promote holistic care coordination.

Qualifications:

  • Education: Master’s degree in Social Work or a related field.
  • Licensure: Preferred license; must progress towards licensure within a specified timeframe. PA Child Abuse Clearance is mandatory.
  • Experience: Minimum of 3 years post-graduate experience, including at least 1 year in a supervisory role.

Required Skills:

  • Excellent verbal and written communication abilities.
  • Proficient in analyzing utilization reports.
  • Knowledge of mental health and substance abuse interventions.
  • Strong problem-solving capabilities.
  • Proficient in MS Office and typing.
