Position: Community Outreach Case Coordinator

Overview:
The Community Outreach Case Coordinator plays a vital role in engaging individuals identified by outreach initiatives and assessing their eligibility and needs based on personal goals.

Key Responsibilities:
  • Conduct assessments to determine individual needs and eligibility for services.
  • Facilitate referrals to community resources and manage ongoing assessment processes.
  • Address long-standing challenges impacting care delivery, community stability, and health promotion.
  • Participate in outreach meetings and maintain communication with team members as necessary.
  • Provide intensive case management services until individuals are connected with permanent care management agencies.
  • Ensure comprehensive transitional care from inpatient settings to other service providers, linking individuals with necessary services upon discharge.
  • Collaborate with other care providers to share information and identify additional care requirements.
  • Monitor service delivery and engage individuals consistently, advocating for further services as needed.
  • Offer transportation to individuals when required, utilizing agency vehicles.

Qualifications:
  • A minimum of one year of experience in case management, including assessment, care planning, and linkage to community services.
  • Ability to work effectively within an interdisciplinary team, particularly in the mental health care system.
  • Knowledge of mental health issues, substance abuse, physical health, and homelessness.
  • A valid and insurable driver's license is required.

Compensation and Benefits:
