Role Summary

The Engagement and Enrollment Specialist plays a crucial role in connecting eligible individuals with Care Coordination services. This position focuses on outreach efforts and member retention for those enrolled in Medicaid and Non-Medicaid programs, particularly for individuals facing co-occurring medical, mental health, and substance use challenges.

Key Responsibilities:
  • Conduct outreach through phone, written communication, and in-person meetings to facilitate enrollment and retention for Health Home eligible individuals.
  • Leverage online databases and health record systems to determine individual eligibility and gather contact information.
  • Maintain accurate clinical documentation in compliance with HIPAA regulations, ensuring confidentiality and thorough record-keeping for all interactions.
  • Assist Care Coordinators in identifying and securing referrals to necessary community and network resources for medical, behavioral health, and social support.
  • Provide follow-up services through telephonic or in-person engagement, coordinating wellness checks and reminder calls as requested.
  • Participate in collaborative meetings and assist with member transportation when necessary.
  • Deliver updates regarding member status, progress, and needs to Care Coordinators and relevant service providers.
  • Complete all required assessments for enrolled members.
  • Engage in community meetings and case conferences as part of the role.
  • Fulfill additional responsibilities as assigned.
Qualifications

Essential Skills and Experience:
  • Bachelor's degree with a minimum of two years of experience working with individuals facing mental health and substance use challenges.
  • Proficiency in electronic health record systems is highly preferred.
  • Ability to travel via public transportation and effectively manage time to meet program and regulatory standards.
  • Experience in outreach, client engagement, assessment, and documentation is required.
