Location: This role is primarily field-based, requiring a balance of remote work and community engagement.

Work Schedule: Standard hours are Monday to Friday, 9 AM to 5 PM, with the expectation of flexibility to meet the needs of families served.

The Outreach Care Specialist plays a crucial role in providing support to individuals and families, ensuring they are linked to essential community services, resources, and care coordination. This position is vital for fostering effective communication among the care team and healthcare providers to support members and their families. Responsibilities include coordinating personalized care plans within the healthcare network and collaborating with various stakeholders to ensure that designated members receive appropriate services. The overarching goal is to enhance independence and quality of life by evaluating individual needs and facilitating access to relevant community services and supports.

Key Responsibilities:

  • Manage follow-up care plans for members by organizing appointments or enrolling them in necessary programs.
  • Evaluate member adherence to medical treatment plans through phone calls or in-person visits.
  • Identify obstacles to compliance and work towards effective solutions.
  • Spot opportunities that influence quality objectives and suggest improvements to processes.
  • Advise on modifications to treatment plans and assess the need for additional services in collaboration with case management and providers.
  • Facilitate identification and referral to local, state, or federally funded programs.
  • Guide members on strategies to mitigate health risks.
  • Compile reports to document case progress and compliance updates.
  • Build and maintain relationships with relevant agencies as outlined in contracts.
  • Perform additional duties as required.

Minimum Qualifications:

  • A high school diploma and at least one year of relevant experience; or a combination of education and experience that provides an equivalent background.

Preferred Qualifications:

  • Familiarity with community resources in the area.
  • Bilingual candidates (Spanish) are preferred.
  • A BS/BA degree in a related field is advantageous.
  • Experience with electronic health records (EHR) systems.
  • For URAC accredited areas, strong oral, written, and interpersonal communication skills, along with problem-solving, facilitation, and analytical skills, are essential.
