About the Role

Health Connect America, Inc. is seeking a highly motivated and compassionate Peer Support Specialist to join our team. As a Peer Support Specialist, you will play a vital role in providing support and guidance to individuals struggling with mental health conditions and substance use disorders.

Key Responsibilities
  • Use your personal recovery experience to connect with and support peers in their recovery journey.
  • Provide education and resources to families and communities on mental health and substance use disorders.
  • Serve as a role model and mentor, promoting a culture of recovery and wellness.
  • Navigate individuals to community resources, such as housing, social services, education, and employment assistance.
  • Lead recovery education and activities, promoting a holistic approach to wellness.
  • Develop and implement Wellness Recovery Action Plans (WRAP) to support individuals in achieving their goals.
  • Facilitate peer support groups, creating a safe and supportive environment for individuals to share their experiences.
  • Provide opportunities for socialization and community engagement, promoting a sense of belonging and connection.
  • Assist individuals in developing and implementing individual service plans, ensuring a person-centered approach to care.
  • Support individuals through the intake process, providing guidance and resources as needed.
  • Educate individuals on changes in healthcare reform and their benefits, promoting informed decision-making.
  • Assist individuals in understanding their benefits and navigating the system to access necessary resources.
  • Support individuals seeking SSI/SSDI benefits through the SOAR process, providing guidance and advocacy.
  • Follow up with individuals after discharge, ensuring a smooth transition and continued support.
  • Empower others by helping them identify their strengths, support, resources, and skills, promoting a culture of self-advocacy.
Requirements
  • Must be at least 18 years old.
  • Must have a mental health condition and/or substance use disorder, be in recovery, and be open-minded.
  • Must be willing to share personal experiences with mental illness/substance use publicly.
  • Must be a High School Graduate or have a GED.
  • Must have a valid driver's license in the state of employment with a safe driving record.
  • Must have successfully completed state-specific Peer Specialist training and exam.
