Client Services Coordinator

The Recovery Support Manager is responsible for planning, executing, and overseeing a comprehensive range of personalized client services throughout their stay and in preparation for ongoing care and secure, sober living post-discharge.
Key Responsibilities:
- Conduct patient evaluations, intake processes, and initial assessments.
- Create or adjust a tailored service plan based on evaluations and additional information; facilitate referrals for necessary services; arrange patient transportation; and monitor referrals during residency and for a minimum of 30 days following discharge.
- Provide one-on-one case management sessions aligned with care level requirements and document the progress in the client's records.
- Support clients in fostering motivation for recovery and participating in recovery initiatives.
- Assist clients in identifying and connecting with suitable resources for immediate needs and subsequent levels of ongoing care, secure housing, and additional recovery support.
- Lead psychoeducational and motivational enhancement group sessions.
- Participate in staff meetings, case discussions, and mandatory training to collaborate with the program team and ensure high-quality care and continuity in line with clinical policies.
- Maintain effective relationships with agencies, community resources, and families to coordinate patient care services; provide information on treatment resources; make necessary referrals; and ensure follow-up to confirm appropriate service delivery.
- Perform other assigned duties as necessary.
Skills and Qualifications:
- Detail-oriented with strong organizational skills.
- Able to work effectively under pressure and meet deadlines while being adaptable and reliable.
- Excellent interpersonal and analytical abilities.
- Outstanding customer service skills with a knack for resolving service-related issues.
- Thorough understanding of industry standards and regulations.
- Capable of applying logic and reasoning to evaluate the strengths and weaknesses of various solutions and approaches.
- Able to manage multiple priorities with a sense of urgency.
Educational Requirements:
- A Bachelor's Degree from an accredited institution, or an equivalent combination of education and experience, is required.
Experience Requirements:
- Proven knowledge and experience in case management is essential.
- Experience in substance abuse, mental health, healthcare, housing, and community support services is required.
- Experience in building rapport with substance-abusing populations and navigating referral and support systems is highly preferred.
- Experience in managing patient services and maintaining both computerized and manual records is strongly preferred.
- Experience in adhering to internal, local, state, and federal regulations is strongly preferred.
